Cuyahoga Development Disabilities Blue Envelope Program Sign-Up

Cuyahoga DD has a limited number of Blue Envelopes to distribute to people we support.

The Blue Envelope Program helps to improve communication and inclusivity between police officers and community members with conditions like autism, dementia, physical disabilities, or other needs that require special understanding during interactions.

The Blue Envelopes can contain the individual’s identification, contact information, or pertinent vehicle operating documents such as the vehicle registration and proof of insurance. The Blue Envelope Program does not have a registry component. Individuals and families choose whether they want to use a Blue Envelope. For more information about Ohio’s Blue Envelope Program, click here. To request a Blue Envelope, complete the form below.

IMPORTANT: Requesting a Blue Envelope does not guarantee you will receive one. We have a limited number.
